Instructions: People's problems sometimes affect their ability to do certain day-to-day tasks in their lives. To rate your problems, select a number from the scale below that best describes how much your problem impairs your ability to carry out the activity.

1. Work

Because of your problem, how much is your ability to work impaired?

2. Home Management

Because of your problem, how much is your home management impaired? (e.g., cleaning, tidying, shopping, cooking, looking after home or children, paying bills)

3. Social Leisure Activities

Because of your problem, how much is your social leisure impaired? (e.g., with other people, such as parties, pubs, outings, entertaining)

4. Private Leisure Activities

Because of your problem, how much is your private leisure impaired? (e.g., done alone, such as reading, gardening, collecting, sewing, walking alone)
