Patient Information

Orientation to Time (5 points)

Ask: "What is the (year) (season) (date) (day) (month)?" Give 1 point for each correct answer.

Orientation to Place (5 points)

Ask: "Where are we: (state) (county) (town) (hospital/building) (floor)?" Give 1 point for each correct answer.

Registration (3 points)

Name 3 objects (e.g., "apple", "table", "penny"). Take 1 second to say each. Ask patient to repeat all three. Give 1 point for each correct answer on first attempt.

Note: Repeat objects until patient learns all three (up to 6 trials). Record number of trials.

Attention and Calculation (5 points)

Serial 7s: Ask patient to count backwards from 100 by 7s (93, 86, 79, 72, 65). Stop after 5 answers. Give 1 point for each correct answer.

Alternative: Spell "WORLD" backwards (D-L-R-O-W). Give 1 point for each letter in correct position.

Recall (3 points)

Ask patient to recall the 3 objects from Registration section. Give 1 point for each correct answer.

Naming (2 points)

Show patient a pencil and a watch. Ask patient to name them. Give 1 point for each correct answer.

Repetition (1 point)

Ask patient to repeat: "No ifs, ands, or buts." Allow only one trial. Give 1 point if successful.

📋 3-Step Command (3 points) - Use Paper

Give patient a piece of paper and say: "Take the paper in your right hand, fold it in half, and put it on the floor."

Score 1 point for each part correctly executed.

Reading (1 point)

Show patient written instruction "CLOSE YOUR EYES" in large letters. Ask patient to read it and do what it says. Give 1 point only if patient actually closes eyes.

✍️ Writing (1 point) - Use Paper

Give patient a blank piece of paper and ask them to write a sentence.

The sentence must contain a subject and verb and be sensible. Correct grammar and punctuation are not necessary.

🎨 Drawing (1 point) - Use Paper

Show patient two intersecting pentagons (each side about 1 inch) and ask them to copy the figure exactly as it is.

All 10 angles must be present and two must intersect to score 1 point. Tremor and rotation are ignored.
